Music Tuition Contract Middle and Senior Students

Use this form to apply for Instrumental, Theory and Vocal tuition programs in 2013.

NOTE: Grade 3, 5 and 7 Introductory Program have a special application form distributed by the teacher.

Music Change Enrolment Form

Use this form to change previous tuition application details in 2013.

NOTE: Only use this form for changes made in the same school year.

Music Instrument Hire Form

Use this form to hire an instrument from the School in 2013.

Music Locker Form

Use this form to hire a locker in 2013. Middle and Senior School students only.
